Program Overview
The PGY2 Ambulatory Care Pharmacy Residency Program at Mercy Health — St. Vincent Medical Center prepares residents to become confident, patient-centered ambulatory care practitioners. Through diverse clinical experiences, residents develop advanced expertise in chronic disease management, evidence-based care and collaboration with interdisciplinary healthcare teams to improve patient outcomes. In a supportive, learner-centered environment, residents receive individualized mentorship while building the confidence and independence needed to thrive as ambulatory care pharmacists.
Direct patient care experiences occur across multiple settings, including provider offices, hospitals, pharmacies, patients' homes and virtual care environments. Through collaborative practice agreements, residents actively manage medication therapy for patients with diabetes, hypertension, hyperlipidemia and anticoagulation needs while helping patients achieve their individual health goals. Residents also gain exposure to pharmacist billing through direct patient care visits and develop the clinical judgment, accountability and independence needed for ambulatory care practice.
The program supports professional growth through experiences that develop teaching, leadership and practice management skills. Residents provide patient education, deliver didactic presentations to medical residents, and precept pharmacy learners ranging from IPPE and APPE students to PGY1 pharmacy residents. Additional opportunities include participation in quality improvement initiatives, interdisciplinary committees and project management activities utilizing Lean Six Sigma principles. For residents interested in academia, an elective academic experience at the University of Findlay allows residents to develop classroom instruction and academic leadership skills.
